Payers have to pay regardless of outcome
– Doctors and healthcare institutions
are paid to perform procedures, not cure illness1. There is no
reward for success, nor any penalty for failure to achieve a successful
outcome.

rewards more costly and invasive procedures, while refusing to reimbursing
therapies that prevent the costly procedures. For example, many
third-party payers will not reimburse a diabetic patient for a $50 visit to a

employers are paying more for inappropriate healthcare for their employees than
they are for appropriate care
(New England
Journal of Medicine March 16, 2006). Fully 45% of healthcare
provided to employees is substandard, regardless of economic status, insured
status, geographical location, or any other meaningful measure. Employers pay anyway. Moreover, “standard” care too
often steers patients into those ineffective, avoidable costly procedures
